It's a cumulative thing afaik. So if you buy 30 days, you get tier 1. Next month you buy another 30 days - 60 days total time. This gets you tier 2. Another 30 days a month later, gets you the tier 3 goodies. They have also said that there are more goodies in store (120 days, 180 days etc.)
^ If it was then that one will likely be the better option. Value for Money wise. (assuming you are planning on playing long term).
My understanding was that your eligibility for the items were based on your total number of paid months. (Kind of a milestone) so if you purchased 180 days you would get these 3 veteran rewards right away on official service start and then the next 3 when they are introduced in the next patch?

90 is the highest "reward" level atm, but they have discussed higher stuff (120, 180 etc.). I haven't looked at account settings, but I'm sure there will be a 6-month option as well for "significant" discount (e.g. $14.99 for 30 days, $14.49 for 60 days, $13.99 for 90 days, $12.99 for 180 days etc.).
And in the live letter, the reveal of this was lead by the question "So, we were meeting with the investors, and they asked about how we are encouraging long-term payment & support." (paraphrasing) So yes, it is cosmetic carrots to ensure long-term, stable income to support development. This idea is something Yoshi P touched on in a letter from the producer a couple months ago, were he discussed P2P vs F2P in general, and why ARR went with a sub model.
